Dementia is the leading cause of death, and people living with dementia often have multiple chronic conditions, making medication management complex. A pilot program funded by Country SA PHN and delivered by the PSA highlights the important role of pharmacists in dementia care. The program provides personalised medication management, education, and advocacy to improve quality of life in rural and regional communities, while supporting families and reducing stigma. Findings show strong collaboration with healthcare teams, enabling deprescribing and better outcomes including improved quality of life. The results also demonstrate a scalable model for safe, patient-centred dementia care.
