Territory Kidney Care (TKC) integrates information from hospitals, primary care, Aboriginal medical services and specialist services, giving clinicians a real time, comprehensive view of a patient’s health journey. By closing information gaps, TKC reduces clinical risk, supports earlier detection of chronic disease, and streamlines decision making for complex patients. As digital health becomes central to future practice, integrated data systems like TKC offer a practical, scalable pathway to safer, more coordinated care.
