This presentation explores the role of the GP Liaison Officer (GPLO) in strengthening the interface between hospital and primary care. Drawing on work from across the NT, it highlights how GPLOs bridge communication gaps and improve care coordination, promote shared care arrangements, and collaborate with NT PHN, Aboriginal health services, and remote clinics. The session offers practical insights into building bridges, not silos, and the value GPLOs add to the system.
